Nurse Case Manager | Branchberg, NJ

The Nurse Case Manager is responsible for assuring the safety, welfare and health of all residents by overseeing and coordinating the medical care of residents within a specified region of New Jersey. The Nurse Case Manager will report directly to the Program Director.


  • Abide by the set forth guidelines of the New Jersey Department of Consumer Affairs – Board of Nursing Statutes 
  • Monitor the daily health of all residents
  • Assist with administration of medications
  • Act as a liaison with physicians’ and carry out physicians’ orders as instructed
  • Assess residents medical needs, determine appropriate medical treatment and initiate the treatment process
  • Maintain complete attention to all residents at the program and place the highest priority on their health, safety, and well being at all times
  • Assist in maintenance of an emergency medical file on each resident
  • Assist in development of medical policies and practices
  • Attend all administrative meetings as directed 
  • Accept on-call status as needed to ensure 24 hour on-call nursing availability for the program
  • Review all incident reports and provide care as needed
  • Oversee all medical care of the individuals served including scheduling all critical/medical appointments and providing escorts
  • Ensure the health and welfare of individuals in the program
  • Maintain appropriate personal appearance and demeanor as a representative of the program and as a role model for individuals and staff
  • Display professional conduct concerning confidentiality with regard to individuals, employees or any aspect of AdvoServ
  • Assist with the development, implementation, documentation and review of behavior treatment Programs utilizing knowledge of residents assigned to him/her in conjunction with the Behavior Analyst or his/her designee(s)
  • Participate in all training sessions as assigned by Program Director or designee
  • Attend at Shift Meetings and other information and educational event.
  • Participate in IHP process for all resident by providing medical information and contributing to goal development
  • Maintain a readiness to interact and/or respond to unexpected Individuals’ behavior in accordance with intervention policies and practices
  • Monitor for actual and/or potential safety issues and take appropriate steps to resolve them in accordance with policy, procedure, and practice   
  • Operate all AdvoServ equipment, appliances, and vehicles in a safe and appropriate manner 
  • Demonstrate the skills, knowledge, and ability to ensure that the program environment is clean and safe, complying with industry standards  
  • Ensure HIPAA compliance 
  • Maintain open and ongoing communication with all other staff, administrators and families as needed
  • Cooperate fully during any inspection or investigation conducted by AdvoServ or its contracting agency, New Jersey Department of Human Services, Division of Developmental Disabilities and any other licensing and regulatory agencies
  • Maintain strict confidentiality with regard to individuals, employees or any aspect of AdvoServ
  • Willing to travel to AdvoServ homes and sites within the region
  • All other duties as assigned

Background and Experience

  • Maintain active New Jersey RN or LPN licensure without discipline against license or privilege to practice 
  • One year of experience in the nursing field
  • Experience working with children or adults with intellectual/developmental disabilities &/or mental health diagnoses preferred
  • Ability to implement crisis intervention techniques
  • Strong desire to work with neurologically-challenged individuals
  • Proficiency in Microsoft Office Suite including Word, Outlook and Excel
  • Ability to provide exceptional client service, demonstrate commitment to continuous learning, display appropriate ethical knowledge and commitment and exhibit a sense of urgency and commitment to quality and timely completion of duties
  • Ability to work in a fast paced environment with changing priorities
  • Ability to interact with staff at all levels in a fast paced environment, sometimes under pressure, remaining flexible, proactive, resourceful and efficient, with a high level of professionalism
  • Ability to organize and prioritize work activities and to handle multiple projects simultaneously
  • Ability to make sound decisions without appreciable direction or advance preparation
  • Strong attention to detail
  • Exceptional organizational skills
  • Excellent verbal and written communication skills 
  • Valid, active driver’s license without any major violations within the past 3 years

Physical Requirements:

Ability to perform CPR (child/adolescent/adult), administer First Aid, and implement approved intervention techniques. Regularly requires walking, standing, stooping, bending, sitting, repetitive wrist/hand movement, and pushing/pulling.  Occasional use of office equipment.  Requires continuous normal hearing (40-60 decibels or 40-60 decibels with correction) and vision (20/20 or 20/20 with correction).  May involve occasional application of approved intervention techniques.  Must be able to lift up to 50 lbs.  Must be free of dependency and abuse of illegal or illicit drugs, alcohol, inhalants, or other chemical substances or controlled substances.  Requires occasional local travel and operation of AdvoServ vehicles.
