The Role

A Home Care Visiting RN provides safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses in PA and NJ.  The RN educates regarding self management and provides support to meet the needs of the patient, family, and community.


  • Conducts a holistic assessment of physiologic, psychological, spiritual, environmental, and cultural traits of patients, families, and the community
  • Performs physical assessments, identifies barriers, prioritizes plans, implements best practice interventions, and evaluates for optimal patient centered outcomes per agency protocols with minimal direction
  • Collaborates, coordinates, and prioritizes services, including physician orders, to meet the needs of patients and families that meet reimbursement criteria
  • Develops plan of care based on assessment findings and holistic needs of the patients and families, adhering to HRHS policies and protocols
  • Notifies physician, and documents all changes in patients conditions, and updates care plans within 24 hours of a visit
  • Accepts direction regarding suggestions for documentation improvement
  • Adheres to infection control and safety policies and practices
  • Performs supervision of CHHA’s at least every 14 days
  • Supervises the care provided by the LPN every 30 days if needed
  • Provides continuity of care and initiates interdisciplinary communication when indicated, observing HIPAA guidelines 
  • Completes mandatory education annually, and inservice training as needed
  • Provides appropriate and timely responses to customer concerns or requests

  • Demonstrates effective, courteous and respectful communication skills

Experience and Background

  • Adheres to HRHS policies regarding time and attendance as well as dress code
  • 1 year med-surg experience required
  • Current licensure in State of practice, Valid Driver’s License, Current CPR Certification
  • Participates in accreditation survey preparedness and related corrective action follow up
  • Is aware of and participates in quality improvement initiatives and infection control plans as necessary
  • Understands and complies with all local, federal, and regulatory body requirements
  • Demonstrates fiscal stewardship by complete accurate OASIS when appropriate
